Résumé : Onze ans après les évènements survenus en Birmanie, John Rambo vit dans l'ancien ranch de son père à Bowie, en Arizona. Il gère les lieux avec Maria Beltran, une vieille amie, et Gabrielle, la petite-fille de Maria. Pensant y rencontrer son père biologique, Gabrielle se rend au Mexique mais elle est kidnappée par un cartel de la drogue mexicain. Rambo tente alors de la sauver.
